A diagnosis of diabetes mellitus is suggested when the fasting whole blood sugar or glucose level is 5.7mmol/L or more or random blood glucose taken two hours after a meal or 75g glucose load (1.7g/kg body weight in children),is 7.8mmol/L or more.
Three common forms of diabetes.
Type 1 diabetes – Formally called insulin-dependent diabetes mellitus or juvenile diabetes
Type 2 diabetes – Formally called non-insulin dependent diabetes mellitus or maturity onset diabetes
Gestational diabetes –Diabetes developing during pregnancy in a previously non-diabetic individual
WHAT IS DIABETES?
Is a disease which develops when glucose or sugar builds up in the body. When food is eaten, it is changed into simple sugar. The main simple sugar is glucose. Glucose is the body’s main fuel for energy normally after eating, glucose is used right away for energy. Glucose that is not used right away is moved or stored into body tissues such as the liver and muscles for later use.
For example, when a meal is missed or delayed, stored glucose is moved back into the blood to be used for energy. Over time, if more food is eaten than needed for energy, the stored glucose will turn into fatty tissue causing the person to gain weight.
Insulin is a hormone made by a gland called the pancreas which lies behind the stomach. Insulin is necessary for life. It helps the body to make proper use of glucose.
Usually in diabetes, the body is not making enough insulin or the insulin does not work properly. This causes the glucose to build up in the blood. When there is too much glucose in the blood, the kidneys pass some into the urine. Normally, there should be no glucose in the urine. Glucose in the blood causes extra water to pass from the body into the urine. This results in the frequent passing of large amounts of urine which leave the person thirsty and dry.
CAUSES OF DIABETES
Defect in the action or secretion of insulin
Environmental factors e.g. excessive calorie intake and lack of physical activity
Genetic factors e.g. Family history, Ethnicity, health etc. Frankly speaking there is no common diabetes cause that fits every type of diabetes. The reason there is no defined diabetes caused is because the causes of diabetes vary depending on the individual and the type. For instance; the cause of type 1 diabetes vary considerably from the causes of gestational diabetes. Similarly, the cause of type 2 diabetes are distinct from the causes of type 1 diabetes.
SYMPTOMS OF DIABETES
No recognizable symptoms in many individuals
Passage of large amounts of urine
Thirst and excessive drinking of water
Unexplained weight loss
Blurred vision
Recurrent boils
Recurrent itching of the vulva
Symptoms related to chronic complications such as ‘pins and needles’ sensation or numbness in the hands and or feet, foot gangrene, poor vision.
SIGNS OF DIABETES
No signs in most patients
Lack of sensation in feet or hands
Foot gangrene
Impaired visual acuity
Cataract
Retinal changes
COMPLICATIONS OF DIABETES
High blood glucose levels over a long period results in chronic complications such as:
Blindness
Kidney failure
Nerve damage
Strokes
Heart attacks
Fertility or pregnancy-related problems
Limb gangrene leading to amputation of the limb
These complications can be prevented through periodic reviews as well as eye and foot examinations accompanied by appropriate investigations
Diabetic coma
Hypogycaemia
SIMPLE TEST AT EARLY STAGES
Taste of sugar in the urine
Urine passed attracts insects such as ants
Use for laboratory analysis and also blood for fasting blood sugar (FBS) or random blood sugar (RBS)
